Clarke/Gray Hockey Waiver

The liability insurance requires that we have a waiver signed by all participants. Please sign the following Spring/Summer 2025 waiver before your first ice session (can't play without it being signed).

In this form there is a signature box for participant (you), you can use the mouse or your finger to make a signature. There is also a spot to type your name, and a spot to fill in the date. Only need to sign the parent/guardian section if the participant is under 18. Here is the link to the waiver:
